About this measure: The PDQ-4+ is a self-report questionnaire developed by Steven E. Hyler, M.D. (Columbia University) based on DSM-IV personality disorder criteria.
It is intended as a screening tool for self-check or group screening, with subscales for twelve personality-disorder-related patterns (paranoid, schizoid, schizotypal, histrionic, narcissistic, borderline, antisocial, avoidant, dependent, obsessive-compulsive, negativistic/passive-aggressive, and depressive).

Instructions:
① There are 107 items. Complete them when you are calm and have enough time. Describe what you are usually like; think about how you have felt, thought, and behaved over the past several years.
② There are no “right” or “wrong” answers—go with your first reaction. Choose one answer per item and do not skip items.
